Submit a Claim - Claim Instructions

To be eligible to receive a share of the Net Settlement Fund in connection with the Settlement of this Action, you must be a Settlement Class Member and complete and sign a Proof of Claim and Release Form (“Claim Form”) and submit it online or mail it by First-Class Mail to the below address no later than March 29, 2024. Failure to submit your Claim Form by the date specified will subject your claim to rejection and may preclude you from being eligible to recover any money in connection with the Settlement.

If you are not a Settlement Class Member do not submit a Claim Form. YOU MAY NOT, DIRECTLY OR INDIRECTLY, PARTICIPATE IN THE SETTLEMENT IF YOU ARE NOT A SETTLEMENT CLASS MEMBER. THUS, IF YOU ARE EXCLUDED FROM THE CLASS (AS SET FORTH IN FAQ 11), ANY CLAIM FORM THAT YOU SUBMIT, OR THAT MAY BE SUBMITTED ON YOUR BEHALF, WILL NOT BE ACCEPTED.

As part of the Claim Form, you will be asked to provide your contact information, detailed information about the losses you incurred, and to provide documentation. Please have this documentation as described above ready, as your Claim will not be saved if you have to come back and finish at a later time.

After submitting your completed claim online, you will receive an email with a Confirmation Code for your completed submission. Receipt of a Confirmation Code means your Claim was successfully submitted. Be sure to keep your confirmation email and code and refer back to them if you have any questions about your Claim Form. If additional information is required to complete your Claim, you will be contacted by the Claims Administrator.

If you wish to submit a Claim Form via standard mail, you may download a copy of the Claim Form here. You will need to provide all the information requested on the Claim Form, attach any supporting documentation, sign it, and date it. Then, mail it to the following address:
